Thank you for your continued trust in Spine & Neuro Center for the care of your patients. We understand that your time is of upmost importance. To simplify the referral process, we have added a downloadable fax referral form. Please complete the referral form and send to our dedicated fax referral line at (256) 533-8082. A Spine & Neuro representative will contact your patient to schedule an appointment within 24 hours at which time the fax referral form will be sent back to your office with date and time of the appointment.

Frequently Asked Questions
How do I get an appointment? You will need a referral from another doctor. Note: if you have Medicaid Patient 1st, you may need to bring a written referral to your first appointment.
How long do I have to wait for an appointment? When a physician refers a patient, the next available appointment is offered for any physician in the group or for a particular physician if requested within one week.
What do I need to bring with me? You need to bring any x-rays, CT, or MRI scans that relate to your problem. Be sure to bring the actual films, not just the report. Also bring the name and dates of other pertinent tests you have had, such as EMG/NCS, carotid Doppler studies, angiograms, etc. You will also need to bring your insurance information and cards.
How long is the usual wait? The average wait to see the doctor is about 30 minutes. Please arrive a few minutes early if you need to fill out paperwork.
Do I need a referral with each visit? If you are already established with the doctor, you may need a new referral if it has been six months since your last visit, or is for a new type of problem. This will depend on the individual case.
Can I change doctors? For continuity of care, our physicians retain the same patients. However, special concessions may be made in individual cases.
Filing Insurance Claims
As a service to our patients, we will file insurance claims to any insurance company for fees generated. You are responsible for any unpaid balance on your account. Please read our policy concerning fees and payments. Although we will file your claim, we cannot be responsible for any claim disputes since we are not a party to your insurance contact.
If your insurance requires a written referral from your referring physician, you must bring this with you at the time of your visit. If you forget to bring the referral your visit may have to be rescheduled.
